It's not ranked according to the global mortality, since flu would have been way higher than no. 7, and CJD would not have made it to top ten. It's also not ranked according to the lethality (probability of death once you get the disease), as many other diseases should have been there, e.g. primary amoebic meningoencephalitis due to Naegleria fowleri - almost 100% fatal. As many others have pointed out, this list also doesn't include debilitating diseases like Alzheimer's, Parkinson disease, clinical depression etc.

Now, most of those people are homeless instead of being treated. Yes, the treatment we have for schizophrenia isn't the best, but it's better to be taken care of than to live on the street.
