The problem w/ Gravatar is that it's sometime slow or down, so your page takes forever to load. If you cache the images yourself, that is more hits on your server when you have high traffic load. Lots of small hits can be nasty.
From what I have read, Gravatars has already been moved onto better servers and is being optimized.
When you have posts with 200 or more comments, loading even 100 small images from your own server is a problem.
You also lose any benefit from browser caching as many people frequent the same blogs.
