        He's a purebred Chinese Crested Hairless dog. When they get old they tend to lose their mane a bit and get wrinkles. Sam just got uglier than most other Chinese Crested Hairless dogs.

It also looks like it is missing most of its teeth, hence the tongue flopping around. Poor little guy. Probably got himself into a bag of candy or something and is all hocked up on sugar.
